Coordinated Services

We collaborate with case managers, private caregivers, and families directly to provide and coordinate a wide range of services. We also partner with other nonprofit organizations to work together to serve more people more effectively without duplicating efforts.

These services are provided for those supported by waivers, other insurances, and private pay options.

Waiver Clients

If we are supporting you and the service is being billed to the county waiver, there is occasionally a patient responsibility portion of your waiver that will we have to invoice and collect. If you receive an invoice, you can mail a check or call with a credit card or pay securely here.

After 20 years in her previous apartment, Kathy got the opportunity to move to a brand new apartment. ConnectAbility of MN helped coordinate a moving service to move her to her new apartment. After Kathy was settled in, she explained that she was in need of some new furniture and household items. Kathy was able to pick out a bright red couch, robin's egg blue, vintage-style toaster and microwave, a beautiful orange shower curtain and rugs, bright red pots and pans, colorful new bedding and rainbow metallic utensils.
