Vendor Responsibility

ConnectAbility of MN serves as a coordinator and facilitator of services but is not the direct provider of those services. As such, we are not responsible for any damages, issues, or liabilities resulting from the actions of third-party service vendors. While we carefully select and partner with vendors to ensure quality and reliability, any concerns or claims related to damage or service quality must be addressed directly with the vendor involved.

CSSP Verbiage

ConnectAbility of MN will assist in securing a service vendor, monitor services as needed, and communicate with the service vendor, case manager and client. Secure and pay the vendor as prescribed by billable service agreement or CSP. ConnectAbility of MN assumes no liability for personal or property damage created by the service vendor.

One Time Chore Services includes:

  • Pest Control

  • Handy man

  • Packing/unpacking

  • Deep clean/ Declutter /Organizing

  • Move in/out clean

  • Tree trimming / brush removal

  • Hoarding cleaning / Biohazard clean up

  • Carpet cleaning

  • Moving furniture

  • Vent cleaning

Transitional Services includes:

  • Essential furniture (e.g., bed frame, mattress, box spring, sofa, dining table and chairs, dressers, lamps)

  • Household supplies (e.g., dishes, drinking glasses, flatware, pots and pans, cleaning products, sheets, towels)

  • Lease or rental deposits

  • Moving expenses (e.g., professional movers, packing, and unpacking services)

  • Pest and allergen treatments for the home

  • Utility set-up fees and deposits

  • Window coverings

Billing Policy: 

We do not partially bill any of the billing code lines. Multi-line billing, takebacks, and rebilling can cause delays, harm to the client, possible loss of services, and multi-patient responsibility charges. 

Our goal is to ensure clients receive the maximum benefit from these services, as they can only be accessed once every three years. 

Using S5121 to help offset extra expenses

To accommodate additional costs, such as high packing expenses, we can utilize S5121 to help cover these costs. 

Transitional Services can only be utilized once every 3 years.

A person can use transitional services to move to their own home or an integrated community supports setting from any of the following eligible settings:

  • Adult foster care home (corporate and family)
  • Assisted living facility that provides customized living services
  • Hospital
  • Institution for mental diseases (IMD)
  • Intensive rehabilitation treatment or rule 36 setting
  • Intermediate care facility for persons with developmental disabilities (ICF/DD)
  • Nursing facility
  • Unlicensed setting (e.g., person’s own home, family home or integrated community supports setting).
Enhancing Client Experience

Our team helps create a smooth and personalized experience by conducting intake calls directly with your client to gather the information we need to best support them. When it’s time to shop, we work closely with the client—often speaking with them in real-time as we select and source items together.

Service Fee

In alignment with MN DHS billing policies, a $650 service fee (T2038) applies. This fee includes:

  • Intake and referral processing, including setting up client records for audit readiness

  • Researching and onboarding vendors to generate accurate quotes

  • Reviewing and processing service agreements and Coordinated Service and Support Plans (CSPs)

  • Ongoing communication with clients, case managers, vendors, and service providers

  • Project coordination, custom product sourcing, and delivery follow-up

  • Billing, invoice processing, and handling product purchases

  • Detailed record-keeping to meet MN DHS audit requirements

  • Personalized follow-up and support for each client
