How do clients get referred to ConnectAbility of MN for services?
Clients are referred to us through their case managers. Our clients are individuals with disabilities who require various support services. Once referred, we coordinate the services and handle the payment on your behalf.
What services does ConnectAbility of MN coordinate?
We coordinate a variety of services for individuals with disabilities, including lawn care, snow removal, general and heavy cleaning, transportation via bus and taxi services, and home modifications through subcontractors. If you have a specific service you provide, please contact us to discuss how it might fit into our program.
How does the process work?
To partner with us, you or your company will need to complete our onboarding process, as required by the Department of Human Services (DHS).
Once the onboarding process is complete:
1. We will reach out to you to obtain a bid for your services.
2. If your bid is accepted, we will notify you when you are approved to start providing services.
Please DO NOT provide ANY services prior to notification from ConnectAbility of MN, as we will not pay for services that were not authorized by us.
What are the requirements to work with ConnectAbility of MN?
Be onboarded with ConnectAbility of MN, you or your company need to:
- Pass a background check
- Pass an OIG exclusion list check
- Sign a HIPAA privacy agreement
- Submit a W-9 form
What is an OIG Exclusion List
The Office of Inspector General maintains a list of individuals who are prohibited from working with vulnerable populations.
Why do I need to provide a W-9 form?
As a non-profit organization, we undergo annual audits. We need to have documentation on file for all vendors we pay for services.
Am I considered an employee?
No, vendors are not employees of ConnectAbility of MN. You are an independent contractor.
How long does the onboarding process take?
The length of time onboarding process varies, but it typically takes 3-5 business days to complete all necessary checks and paperwork. We will notify you when the process is complete or if we need additional information.
Do you withhold taxes?
No, those we partner with are considered independent contractors. We do not withhold any taxes. You should consult a tax professional to understand your personal tax obligations.
What are my responsibilities as an independent contractor?
As an independent contractor, you are responsible for managing your own schedule, maintaining your own liability insurance, and ensuring that you comply with all relevant laws and regulations. You should also keep accurate records of the services provided and submit invoices in a timely manner.
How do I get paid?
After you have been approved to start providing services, please send a monthly invoice to finance@ConnectAbilityMN.org. Your invoice should include the client’s name, the service provided, and the date of service. If you need an invoice form, we can provide one for you via mail or you can download it from this page by clicking the “Blank Invoice” button.
Additionally, please note that under no circumstances should you collect money from a client or discuss billing matters with them directly. This ensures compliance with our payment procedures and client confidentiality standards.
What information should I include on the monthly invoice?
Each invoice should include:
- Client’s full name
- Client’s address (if applicable)
- Date of service
- Description of service provided
- Invoice date
- Invoice total
- ConnectAbility of MN as they payer (not the client or Service Coordinator you communicate with)
- Your or your company’s name and address as it appears on the W-9
If necessary, multiple clients may be listed on the same invoice.
Should I include sales tax on my invoices?
No, as a non-profit organization, we do not pay sales tax. A current ST3 is available on this page for your records.
Where does your funding come from?
We are a non-profit organization working with the Department of Human Services to coordinate services for people with disabilities in Minnesota. When you send us an invoice for services, we bill the state and then pay you.
How much can I charge?
Please quote your typical rates for similar services. Once you provide your pricing, we need to get funding approval before you can start providing the service.
What happens if there is an issue with payment?
If there is an issue with payment, please contact our finance department at finance@ConnectAbilityMN.org. We will work with you to resolve any issues as quickly as possible.
Can I provide services to multiple clients through you?
Absolutely! You can provide services to multiple clients referred by us.
What if I need to change my service rates?
We understand that the cost of doing business can fluctuate. However, due to how individual clients’ funding is structured, we may not be able to adjust pricing until their service plan is up for renewal. If you provide ongoing services, we will contact you for a new bid when the client’s plan is due for renewal. This typically occurs annually, though the specific date varies for each person we serve. You can submit a bid with your updated pricing at that time.
Will ConnectAbility of MN sign a contract for services?
Due to the variability of individual funding situations, we do not sign contracts for services. If there are any changes to a person’s eligibility, we will notify you immediately.
What should I do if I have a conflict with a client?
If you experience a conflict with a client, please contact us immediately. We will work with you and the client to resolve the issue in a fair and timely manner.
All communication should go through ConnectAbility of MN to ensure accurate information is conveyed and only approved services are authorized. This approach is crucial to ensuring clear communication among all involved parties.
Can I decline a client referral?
Yes, you can decline a client referral if you feel that the service request does not fit your capabilities or availability. Please notify us as soon as possible if you need to decline a referral.
What if I need to take a leave of absence or stop providing services temporarily?
If you need to take a leave of absence or stop providing services temporarily, please inform us as soon as possible. We will make arrangements to ensure that your clients continue to receive the necessary services during your absence.
How do I handle confidential information?
As part of the onboarding process, you will sign a HIPAA privacy agreement, which outlines how to handle and protect confidential client information. It is essential to adhere to these guidelines to maintain client confidentiality. Personal information, such as names, addresses, or individual circumstances, should never be shared with parties not directly involved in providing the service. This ensures that client privacy is rigorously upheld and respected.
How do I update my contact information?
If you need to update your contact information, please notify us in writing. We will update our records accordingly to ensure we can reach you as needed.
