Our Transitional Services support individuals in moving from care facilities to independent living. We cover necessary items and expenses to help ease the transition, such as household furnishings, moving costs, and essential supplies. Our goal is to make the process smoother and ensure individuals have the resources they need to succeed in their new homes.

We are currently servicing ALL Minnesota counties.

What are Transitional Services?

Transitional Services cover essential items and reasonable expenses to help individuals move from an eligible care setting to a more independent living arrangement, such as their own home or an integrated community support setting. These services ensure a smooth and supportive transition by addressing critical needs like household furnishings, moving assistance, and basic supplies required for independent living

What Transitional Services are Available?

Transitional services provide essential resources to support individuals moving from a care setting to independent living.

These services include:

  • Payment of required deposits, such as for rent or utilities.
  • Provision of essential furniture, household items, and kitchen supplies.
  • Professional packing, moving, and setup services.
  • Assistance with fees related to utility setup and home safety.
  • Support for a smooth and timely transition into the new living space.

What is the Process for Transitional Services?

  1. Submit a referral as the case manager.
  2. Our team will find a vendor, secure a bid, and send you a quote for the service agreement.
  3. Once ConnectAbility of MN receives a service agreement from the State of Minnesota, the service can begin.

Things to Consider:

 

    • Required lead time: 2–4 weeks
    • Answer all questions thoroughly
    • Provide as much detail as possible when completing the referral
    • Send pictures of items to be packed and moved.
    • Transitional Services does not cover rent, food, or clothing
    • Indicate if the client is using CDCS.
    • Accuracy is crucial—timing matters with moves. Incorrect information can cause approval delays of several weeks if adjustments are needed after approval
