The Transitional Services team at ConnectAbility of MN assists those moving to their own home or integrated support setting. We do everything to ensure the move goes as smoothly as possible and that the client has essentials in their new home.

What are Transitional Services?
Items and expenses necessary and reasonable for a person to transition from an eligible setting to their own home or an integrated community support setting.

What Transitional Services are Available?
A qualified candidate can have their items packed and moved, have their required deposits paid for, and receive an allotment for furniture and household supplies to get them started in their new home.
What is the Process for Transitional Services?
As a case manager, you will need to start by submitting a referral. When the referral is received, the team finds a vendor to do the job and secures a bid for the job. A quote is created and sent to the case manager to submit into their system. When ConnectAbility of MN receives a service agreement from the state of MN, the service can proceed.
Things to Consider:
The required lead time is 1 – 3 weeks.
Answer questions fully and completely.
When completing the referral give as much detail as possible.
Send pictures of what will be packed and moved.
List full deposit contact information.
Transitional Services does not pay rent, food, or clothing.
Is the client CDCS?
With a move, timing is critical. It is important that all the information given is correct. The approval can be delayed for weeks when the amounts need to be adjusted after the approval.
